1963年東京都生れ。88年千葉大学医学部卒業。東京都立府中病院、千葉大学医学部附属病院を経て、2001年より東京厚生年金病院呼吸器内科医長。07年より内科部長兼地域医療連携室長。日本内科学会認定内科専門医、日本プライマリケア学会認定医、日本旅行医学会理事。
「何かつらいことはありますか？」だけではなく
「痛みはありますか？」と具体的に訊ねる
「肺がんの痛みには２種類ある。一つは骨への転移や神経への侵潤、もう一つは胸膜という肺を包んでいる膜や胸壁への浸潤によるもの。いずれもステージ３ｂから４にかけての進行がんで見られる症状です」
溝尾医師が担当する患者は、手術の適用範囲からは外れ、抗がん剤による化学療法の対象となったケースだが、それでも痛みの出方には個人差が大きいという。
「ボルグスケールという痛みや疲労感の尺度を測る指標で判断します。10段階のうち、今の痛みがどの程度かを申告してもらうのですが、それでも人によって我慢してしまう人はいる。ただ、これは医師や看護師の側の接し方で、本当の痛みを聞き出すことは可能。『何かつらいことはありますか？』と訊くだけではなく『痛みはありますか？』と具体的な質問をすることで、患者さんは正確な痛みを話してくれるものなんです」
このあたりは、溝尾医師が臨床経験から編み出していった知恵であり、こうした細やかなテクニックの積み重ねで、患者のＱＯＬは大きく左右されるのだ。
治療最前線
ＥＢＭだけに捉われることなく――
ＱＯＬを維持しながらの肺がん治療を実践

俳優の柴田恭兵さんが手術を受けていたことで、にわかに注目を集めている肺がん。早期発見が難しいことから「治療難易度の高いがん」と言われてきたが、特効薬「イレッサ」の登場などもあり、近年は少しずつ治療成績も向上している。そんななか、手術の適用外となる３ｂ－４期の肺がん治療において数多くの症例を持ち、免疫力向上を目的とした在宅治療や「旅行」を取り入れたグループ治療など、新たな取り組みに力を入れているのが、東京厚生年金病院内科医長の溝尾朗医師だ。そこで瀬尾医師に、取り組みの内容と今後の抱負を語っていただいた。
経験的に積み重ねられた傾向から
ＱＯＬの高い治療を実践――
溝尾医師のもとを訪れる外来患者さんは、１日あたり約60人。そのうち肺がんの患者さんは10人ほどで、いずれも手術適応から外れた「３ｂ期」か「４期」のステージに該当するケースだ。基本的な治療は、化学療法をメーンに、状況に応じて放射線治療を組み合わせるオーソドックスなもの。しかし溝尾医師は、極力入院を避けて在宅での治療をめざしたアプローチを模索する。
「これは治療法というよりは、私自身の信念なのですが、単にEBM（根拠に基づいた治療）にそって坦々と治療を続けるのではなく、“ＱＯＬ（生活の質）を維持する”という点にこだわりながら、効果的な治療を続けていきたいと考えています。私のこれまで診てきた患者さんの傾向を考えると、例えば『食欲のある人はない人より治療効果が高い』、あるいは『行動範囲が広い人のほうが狭い人よりも治療実績がよい』といった、EBMの確立までは至っていないものの、経験的に積み重なってきた傾向がある。それに沿った治療を行うことで、実際に治療の上でもよい成績につながっているし、患者さんにも喜ばれています
喫煙習慣を「単なる個人の嗜好の問題」とせず，ニコチン依存症という立派な（？）疾患とみなして公的に治療しようというものだが，どのような治療が受けられるのか，成功率は？ 費用は？ など色々と気になる点も多い。
そこで，東京厚生年金病院で「禁煙外来」を担当する呼吸器科・溝尾朗先生に，禁煙治療の実際を聞いてみた。
溝尾先生によれば4月以降に「禁煙外来」を受診したのは約50人。「まだ期間が短いので，最終的な禁煙成功率は出ていませんが，だいたい8割の方が禁煙を継続されています」という話だ。ちなみに，先生の外来を受診した最高齢は92歳。「70年間，日に50本喫煙していたそうです。スーッとやめられましたよ」

昭和61年福建中医学院大学卒業、同大学助手
平成元年来日、平成2年から平成7年 名古屋大学医学部研究生、大学院生
平成7年株式会社薬日本堂就職、漢方相談専門職
平成21年株式会社黒姫和漢薬研究所に勤務
中国福建省出身の「中医師」。大学卒業後、福建中医学院（2009年9月から福建中医薬
大学に名称変更）大学助手として附属病院整形外科で生薬を活かし臨床実践。中国の
「中医薬出版社」から出版された「中国医学非薬物療法」編集者として参加。
平成元年に東洋医学と西洋医学を融合するために来日、以後平成2年からの5年
間、名古屋大学医学部研究生・大学院生として漢方と自律神経について学び、
その間に名古屋市の医師会や製薬会社の協力で、医師向けの漢方勉強会を実施。
その後、国内大手漢方相談薬局に15年間勤務。難病から風邪予防の数万人の健康相談
を受け、心と身体の健康を願い漢方療法の実践に務める。また、テレビや新聞・雑誌
等に健康情報を10数年間発信し、社会貢献活動を行う。平成21年10月から、和漢草成
分で健康サプリメントを研究開発するため、株式会社黒姫和漢薬研究所に入社。
演題：和漢薬について－中国の伝承医学と日本の民間薬の融合
【内容】
第一部 中国の医療現状
１．伝承医学
1956年に北京、上海、広州、成都に中医学大学から始め、現在では32個大学（西洋
医学は104個）、最初は5年間でした、今は7年間制です。
２、医療の現状
病院は3種類があり、総合病院、中西結合病院、中医病院，総合病院では中医内科、
中医小児科など中医の専門診療部門
３、患者さんの選択
診療は患者さんの自身の選択の自由です。
第二部 日本の和漢草は多様化
１、民間薬とは古くから一般に言い伝えられ、利用されてきた薬草です。
代表的な薬草は
① 延命草
② センブリ
③ ゲンノショウコ
④ ドクダミ
⑤ エビスグサ
⑥ ハトムギ
⑦ ヨモギ
２、エキス剤の商品化 1957年に最初に漢方エキス製剤の商品化し、1967年に葛根湯、
五苓散などの4種類は保険適用として応用し始めます。利便性で普及しやすい。ま
た、吸収性、抽出率、原料の有効利用と環境に優しいことから、これからも市場
拡大と予想されます。
３、私の実践
①現代医学と漢方の併用で、クオリティ・オブ・ライフ（生活の質）を高めて、病気
と共存しそれを克服していく一つの有力な方法の実例平成8年の夏に、腎臓ガンで
肺に転移し、肺炎も併発し、高熱が続いています、近くの墓石店に行く帰りに寄
りました。「2週間余命と言われ、何かいいものがありますか」と相談を受けまし
た。まず、心の持ち方を変えるとアドバイスし、三七人参花含有健康食品を紹介
しました。2週間目で平熱になり、食欲が出ました。1年後に、検査の結果では、
肺ガンが小さくなったと本人様が嬉しく報告。平成22年現在でも電鉄の駅員とし
て、元気で勤務しています。

He received his B.S. in Oriental Medicine in 1981, M.S. in 1983, Ph.D
in 1987 from Graduate School of Kyung Hee University and accomplished
the Master course of Chinese Philosophy in 1989 from Graduate School
of Korea University.
He became a full time lecturer of Daejon University in 1985 and moved
to Kyung Hee University as an Assistant Professor in 1988 until 2003.
He was invited as a visiting professor by China Medical College and
National Science Council of Taiwan Government in 1989 and 1990
consecutively, State Administration of Traditional Chinese Medicine,
China in 1993, and Medical School of Stanford University, USA in 2001.
He served as Vice President of Korean Oriental Medical Doctor's
Association in 1996, Chairman of Oriental Medical College in Kyung Hee
University in 1998, President of Korean Society of Oriental Oncology
in 1999 and Vice President of Korean Oriental Medical Society in 2002.
In 2004, he was appointed as Regional Adviser in Traditional Medicine,
WHO Regional Office for the Western Pacific. During his tenure in
WHO/WPRO, his work mainly focused on standardization of various areas
of traditional medicine in the Western Pacific Region, such as
acupuncture point locations and terminologies on traditional medicine.
He spearheaded these projects which are considered milestones in the
history of traditional medicine in the Region. He published 11
monographs and is author and co-author of 120 papers. In August 2008,
he assumed the post of Dean, College of Oriental Medicine, Kyung Hee
University. Currently, he is working as the President of Global
University Network of Traditional Medicine, the President of Korean
Council of Deans of Oriental Medical Colleges, the President of Korean
Society of Oriental Pathology and a Member of Council on Social Policy
Consultation, Office of President, Rep of Korea.

In recent years Oriental medicine has been spreading in western countries.
It has ranged from a complementary to Western medicine to an alternative
medicine (CAM); currently, efforts to integrate the two are being made.
Locally, the revision of Korea's Medical Practice law on January 8, 2009
enabled local medical institutions at or above the hospital level to start
collaborative clinical practice integrating Oriental and Western medical
services as of January 31, 2010. In the wake of those local and overseas
developments a number of Korea's university-affiliated hospitals of Oriental
medicine have started with earnest efforts to integrate East-West clinical
services. Kyung Hee University has been playing a leading role in introducing
a new model of medicine in line with the integral harmony of East-West medical
collaboration.
Using as our guiding principle, our mission to improve quality of life, we
need to create a human-oriented medical culture with the emphasis on the newly
re-established concept of good health.
Towards this end, we need to create a new model of medicine in line with the
harmonization of holistic perspectives of Oriental medicine and analytic and
reductionistic approaches of Western science of life and medicine.
As a practical way of realizing our vision, an ideal East-West Collaborative
Clinical (EWCC) model will be developed so as to provide high-quality medical
service satisfaction of the patients. In the process, coordinated efforts and
research will be sought in various fields including Oriental medicine, Western
medicine, natural sciences and the Humanities. Related basic research will be
simultaneously conducted in finding methods for greater and more effective
integration.
Upon development of the EWCC model, a series of follow-up actions will be
needed for the groundwork of the research base: a communication network to
facilitate successful implementation of the EWCC model, system improvement, a
mapping-out of the management strategy, studies on herb-drug interactions,
modern interpretation of Oriental medicine theories, and the construction of
related databases. As with most successful undertakings, a well-thought out,
solid foundation is a prerequisite.
Proposed Kyung Hee Institute for Neo-Medicine will be used to create that
foundation for the harmonious coexistence of both Western and Oriental
medicine, aiming at an integrative medicine, which should be achieved for the
benefit of all peoples.

演題：「奇跡的治癒とphysis (自然力)－エピソードの意味論－」
【内容】
「末期ガンからの生還」といいた記事が健康雑誌などによく見られる。「奇跡的治癒」
などと表現されることもある。が、このようなケースは「奇跡」というなど珍しいこと
ではなく、実際にはり多いように思われる。 たいていは、いわゆる「標準治療」のあ
げく、余命を宣告された患者さんが「告知期間」をはるかに超えて延命し、あるいは
「奇跡的治癒した。」という話である。
一般にこのような体験談は、通常医学の側からは、単なる再現性のない「エピソード」
に過ぎないとして、資料性を否定される。しかし、「エピソード」を子細に検討してみ
ると、年齢、性別、環境、生活習慣の異なる「奇跡」の主人公たちの治療の過程にはあ
る種の「共通項」がみられる。もしかしたら、ここにガンの本質に関わる何かが隠され
ているのではないか。「エピソード」の意味の実存的解析が病気の本質、治療のヒント
を与えてくれるのではないかというのが小論の趣旨である。
最もこれまでにも、クレスチンなど「エピソード」や「伝承」から出発して創薬され
た新薬も少なくはない。そしてこれらの「奇跡的治癒」を引き起したエピソードの主人
公たちの生命の復元力・ホメオスタシス維持能力の本質は、ヒポクラテス以来の自然の
力なのではないのか。ということを1、2の具体例を挙げて検証してみたい。合わせて、
今日通常医学で行われている余命告知の問題を指摘し、神を恐れない所業に対する猛省
を促したいと考えるものである。

演題：「西洋医学を補う漢方の知恵と哲学」
【内容】
日々、次々と進歩する現代医学によって私たちは非常に大きな利益を得ました。しかし、
限界もあります。
漢方はブームともいえる注目を集めていますが、それゆえ誤解、不信、先入観で捉えら
れる場合も少なくありません。
融合医学の為には、漢方医学の本当の“姿”を知ること、知らせることが、これから大
切になると思います。漢方医学は漢方薬（くすり）だけではなく、その中で病気になら
ない為の知恵、健康に生きる為の知恵、人生のあり方や生命の意味までの哲学がありま
す。簡単にご紹介します。
①自然観と整体観
自然と人とは常に一体になって溶け合って動いています。人体は一個の有機体整体
であり、調和しながら健康を保っています。
②陰陽の考え方
自然、人、生命、宇宙万物を理解する為、先人が作り上げ伝えてくれた知恵があり、
健康法もあります。
③気・血・水理論
病気や異常の原因と治療法と密接に関係しています。
特に“気”の正体 “気”の働きはとても大切な一部になっています。
④未病を治す
未病とは病気をなる一歩手前の状態の事です。漢方ではもともと重要な部分で、多
数の方法論や処方があります。
⑤漢方の診察（四診）
望・聞・間・切法により、五感を研ぎ澄ませてその人にぴったりあったオーダーメ
イド医療法です。
⑥医食同源
今や、思い出される生活の中に溶け込んだ生活医学。病気しない為、薬に頼らない
為、“健康で幸せに生きる”為の生活の知恵があります。
まとめ：
融合医学の為には、西洋医学にはない知恵が必要です。
古くから伝わる漢方医学の知恵を最先端の西洋医学にプラスすることで“融合医学”
は実現出来ると思います。
